If you haven't heard: what we've long called PCOS (Polycystic Ovary Syndrome) was recently renamed PMOS, Polyendocrine Metabolic Ovarian Syndrome. Same condition, same diagnosis, just a name that finally reflects everything actually going on in your body.
Here's the good news buried in that name change: for years, this condition got boiled down to "cysts on your ovaries." But that was never the full story, and it left a lot of people feeling confused and unheard. PMOS makes it official this is about your whole system, especially your metabolism. Which is something you have a lot of power to influence.
If you've been managing this with birth control, spironolactone, or being told to "just lose weight," and it hasn't made you feel like yourself again you're not doing anything wrong. Those approaches often treat the symptoms such as irregular cycles, acne, excess hair growth, weight changes without ever addressing what's driving them: insulin resistance.
What PMOS Actually Is
PMOS is typically diagnosed when someone has at least two of the following three:
- Excess androgens (elevated testosterone, showing up as acne, hair loss, or hirsutism)
- Ovulatory dysfunction (irregular or absent periods)
- Metabolic dysfunction (insulin resistance, weight gain, blood sugar swings)
The metabolic piece is usually the driving factor, not just one symptom among three. Once you understand that, everything about how to feel better starts to make a lot more sense. When insulin resistance is in the picture, your body compensates by producing more and more insulin to get the same blood sugar lowering effect. That extra insulin doesn't stay in its lane, it ripples outward and affects your hormones too. It signals your ovaries to produce more testosterone, lowers SHBG (the protein that binds up testosterone), leaving more of it "free" and active in your body. It disrupts the signaling needed for regular ovulation and drives inflammation, which further worsens insulin resistance.
In other words: insulin resistance drives excess androgens which disrupts ovulation and drives the cascade of symptoms.

The root cause is still running quietly in the background because insulin resistance is so central to this picture, it's also one of the most responsive things to lifestyle changes. Where to Start: Blood Sugar Regulation
If insulin resistance is the engine driving PMOS, then blood sugar regulation is where healing starts. None of this requires perfection, a few consistent foundational shifts make an outsized difference. Aim for at least 100g of protein per day distributed across evenly portioned meals. Include a source of healthy fat at each meal (this slows glucose absorption and blunts insulin spikes.) Reduce insulin drivers: frequent snacking, added sugars, and highly processed/inflammatory foods.
Movement
Exercise causes your muscles to pull glucose out of your bloodstream via GLUT-4 transporters, without needing extra insulin to do it. This is why resistance training is one of the most powerful tools for improving insulin sensitivity: muscle acts as a sink for glucose. More muscle means more GLUT-4 activity, so building strength directly improves blood sugar regulation over time. Something as simple as a short walk after meals can be effective, even 10-15 minutes measurably blunts the post-meal blood sugar spike.
Gut Health
This one's easy to overlook, but it matters more than most people realize. Your gut microbiome influences hormone metabolism (including how estrogen is processed and cleared) and is a major contributor to systemic inflammation, which feeds right back into insulin resistance. Supporting your gut can be a quiet but powerful piece of the puzzle.
Get the Full Picture
Standard bloodwork often misses the markers that actually explain what's driving your PMOS. When you address the metabolic root the downstream hormonal chaos has a real chance to settle, not just quiet down temporarily. Beyond a traditional panel, it's worth asking your provider to include:
- Fasting insulin: often the earliest sign of insulin resistance, showing up abnormal well before fasting glucose does.
- Fasting glucose: a baseline measure of how well your body regulates blood sugar.
- Homocysteine: reflects how well your methylation pathways are working and can flag underlying inflammation.
- CRP (C-reactive protein): a key marker of systemic inflammation in the body.
